Abstract

Latar Belakang : Psoriasis merupakan penyakit peradangan kulit kronik residif didasari oleh reaksi imun yang disebabkan oleh faktor genetik, imun, perubahan biokimia, efek lingkungan, trauma, infeksi, endokrin, stres emosional, obat-obatan, alkohol, merokok dan stres oksidatif. Stres oksidatif merupakan proses fisiologis alami dalam sistem biologis dimana adanya radikal oksigen bebas yang melebihi mekanisme pertahanan radikal (radical scavenging). Hal ini menyebabkan ketidakseimbangan antara oksidan dan antioksidan. Oksidan dapat berasal dari reactive nitrogen species yaitu nitrit oksida yang bersifat radikal. Superoksida dismutase merupakan antioksidan yang berperan dalam menetralkan radikal bebas. Tujuan : Mengetahui hubungan rasio nitrit oksida dan superoksida dismutase serum pada pasien psoriasis vulgaris berdasarkan derajat keparahan penyakit. Subjek dan metode : Suatu penelitian cross sectional yang dilakukan pada pasien yang telah didiagnosis sebagai psoriasis vulgaris di Poliklinik IKKK RSUP Dr M Djamil Padang. Jumlah subjek penelitian sebanyak 34 orang dan tidak ada sampel yang drop out. Rasio nitrit oksida dan superoksida dismutase serum diperiksa dengan enzyme linked immunosorbent assay. Penelitian dilakukan dari Februari-Juni 2016. Hasil : Terdapat korelasi positif dengan kekuatan lemah antara rasio nitrit oksida dan superoksida dismutase serum dengan derajat keparahan psoriasis vulgaris berdasarkan skor PASI. (r = 0,171, p =0,334). Kesimpulan : Pada hasil penelitian ini didapatkan rerata kadar nitrit oksida serum pada pasien psoriasis vulgaris meningkat dari kadar nitrit oksida normal serum, rerata kadar superoksida dismutase serum pada pasien psoriasis vulgaris menurun dari kadar superoksida dismutase normal serum. Rerata rasio nitrit oksida dan superoksida dismutase serum meningkat dari rasio nitrit oksida dan superoksida dismutase normal serum. Namun rasio nitrit oksida dan superoksida dismutase serum berdasarkan derajat ringan, sedang dan berat tidak terdapat perbedaan yang bermakna secara statistik. Tidak terdapat hubungan yang bermakna antara rasio nitrit oksida dan superoksida dismutase serum dengan derajat keparahan psoriasis vulgaris. Kata kunci : rasio nitrit oksida dan superoksida dismutase serum, psoriasis vulgaris, derajat keparahan (skor PASI) Abstract Background : Psoriasis is a chronic inflammatory disease of the skin residif constituted by an immune reaction caused by genetic factors, immune, biochemical changes, environmental effects, trauma, infection, endocrine, emotional stress, drugs, alcohol, smoking and oxidative stress. Oxidative stress is a natural physiological processes in biological systems where the presence of free oxygen radicals that exceed defense mechanism radical (radical scavenging). This creates an imbalance between oxidants and antioxidants. Oxidants can be derived from reactive nitrogen species is nitric oxide radical. Superoxide dismutase is an antioxidant that plays a role in neutralizing free radicals. Objective: To asses the correlation between nitric oxide/superoxide dismutase serum and severity index score of psoriasis vulgaris patient. Subject and methode : A cross-sectional study conducted in patients who had been diagnosed as psoriasis vulgaris in outpatient Dermato-Venereology Clinic of Dr. M. Djamil Hospital Padang. The total sample is 34 patients and there was no drop out. The ratio of nitric oxide/ superoxide dismutase serum was analyzed using enzyme linked immunosorbent assay. The study was conducted from February to June 2016. Result: A weak positive correlation was found between ratio nitric oxide/ superoxide dismutase serum and severity index score of psoriasis vulgaris (r = 0. 0,171, p =0,334). Conclusion : On the results of this study, the mean serum levels of nitric oxide in psoriasis vulgaris patients increased from normal serum levels of nitric oxide, the mean serum levels of superoxide dismutase in psoriasis vulgaris patients decreased of normal serum levels of superoxide dismutase. The mean ratio of nitric oxide and superoxide dismutase increased from ratio of serum nitric oxide and superoxide dismutase normal serum. However, there is no statistically significant difference between the ratio of nitric oxide and superoxide dismutase serum in mild, moderate and severe. There was no significant correlation between the ratio of nitric oxide and superoxide dismutase serum and severity of psoriasis vulgaris.
