Abstract
The Ministry of Health of the Republic of Indonesia launched the Sehat Indonesiaku (ASIK) Application as a foundational step in the digital transformation of the national health system, integrating it into the broader health transformation agenda. This application serves as a digital platform for recording and reporting, including routine immunization data, in all primary healthcare facilities nationwide. However, its implementation at Public Health Centers (Puskesmas) in Padang City remains suboptimal, evidenced by persistent discrepancies between manual and digital reports and the failure to meet immunization coverage targets. Therefore, this study aims to identify the factors associated with the performance of health workers in utilizing the ASIK application at these centers. The research employed a mixed-methods design, combining a quantitative cross-sectional study with total sampling and a qualitative study using in-depth interviews with thematic analysis. Quantitative findings from the chi-square test indicated significant associations between health workers' performance and motivation, leadership, and workload. Multivariate analysis revealed motivation as the most dominant factor. Qualitative findings identified seven key themes: complex motivational dynamics, primarily driven by professional responsibility; limitations in facilities (use of personal devices and internet); application functional issues (e.g., errors and data loss); increased administrative workload; the digital generation gap; adaptive strategies developed by health workers (time management and team collaboration); and a recommendation for using a single integrated application for recording and reporting in primary healthcare facilities. The study concludes that improving performance in ASIK utilization requires comprehensive interventions focusing not only on technical aspects but also on strengthening intrinsic motivation, providing adequate facility support, and improving the application system.
