Abstract
Background: Hyponatremia in traumatic brain injury could cause water shifting into glial cells and neurons leading to brain edema, herniation and mortality. Previous studies have shown a high rate of decreased consciousness in head injury patients with minimal bleeding lesions accompanied by hyponatremia. However, there is still few evidences regarding this in West Sumatra Objective: To assess the effect of the degree of hyponatremia on the severity of traumatic brain injury in head injury patients who underwent conservative management at Dr. M Djamil General Hospital Padang in 2023-2025. Methods: This is a quantitative study with a cross-sectional design conducted at the Dr. M. Djamil General Hospital Padang in January 2023 to January 2025. All head injury patients with hyponatremia who underwent conservative management were included in the study. Patients with comorbidities, multiple fractures, who underwent operative management based on imaging and inoperable head injuries were excluded from the study. Results: There were 112 study subjects. The average age of patients was 46.69 ± 17.95 years, most were male (74.1%), experienced mild hyponatremia (83.9%) and experienced moderate head injury (60.7). In the severe hyponatremia group, 16.7% experienced severe head injury and 83.3% experienced moderate head injury. In moderate hyponatremia, 41.7% experienced severe head injury, 25% experienced moderate head injury and 33.3% experienced mild head injury. In mild hyponatremia, 20.2% experienced severe head injury, 63.8% experienced moderate head injury and 16% experienced mild head injury. This study did not find an effect of the degree of hyponatremia with the severity of head injury (p = 0.392) Conclusion: This study did not find an effect of the degree of hyponatremia with the severity of head injury.
