Abstract

HUBUNGAN KADAR VITAMIN D, ENZIM ALKALINE PHOSPHATASE DAN ENZIM MATRIX METALOPROTEINASE-20 DENGAN KEJADIAN ENAMELDEFECT GIGIPERMANEN PADA ANAK Reno Wiska Wulandari ABSTRAK Pendahuluan:Enamel merupakan jaringan paling keras dalam tubuh manusia yang pembentukannya melibatkan interaksi antara protein matriks, enzim, dan proses mineralisasi. Vitamin D berperan dalam homeostasis kalsium dan fosfat, ALP berperan dalam proses mineralisasi, sedangkan MMP-20 berperan dalam pemrosesan protein matriks enamel. Gangguan pada proses biologis tersebut dapat berkontribusi terhadap terjadinya enamel defect. Penelitian ini bertujuan untuk menganalisis hubungan kadar vitamin D, ALP, dan MMP-20 saliva dengan kejadian enamel defect pada gigi permanen anak. Metode: Penelitian ini menggunakan desain cross-sectional comparative pada 60 anak berusia 6–13 tahun di wilayah kerja Puskesmas Lubuk Kilangan, Kota Padang. Subjek dibagi menjadi kelompok enamel normal dan enamel defect, masing-masing sebanyak 30 anak. Kadar vitamin D dan MMP-20 saliva diperiksa menggunakan metode ELISA, sedangkan ALP diperiksa menggunakan metode kolorimetri. Data dianalisis menggunakan uji Mann–Whitney. Hasil: Terdapat perbedaan bermakna kadar vitamin D (p=0,001) dan MMP-20 (p<0,05) antara kelompok enamel normal dan enamel defect, sedangkan ALP tidak menunjukkan perbedaan yang bermakna (p=0,641). Kesimpulan: Kadar vitamin D dan MMP-20 saliva berhubungan secara signifikan dengan kejadian enamel defect pada gigi permanen anak, sedangkan ALP tidak menunjukkan hubungan yang signifikan. Vitamin D plays a role in calcium and phosphate homeostasis, ALP is involved in the mineralization process, while MMP-20 plays a role in the processing of enamel matrix proteins. Disturbances in these biological processes may contribute to the development of enamel defects. This study aimed to analyze the association of salivary vitamin D, ALP, and MMP-20 levels with enamel defects in children’s permanent teeth. Methods: This study used a cross-sectional comparative design involving 60 children aged 6–13 years in the working area of Lubuk Kilangan Public Health Center, Padang, Indonesia. The subjects were divided into normal enamel and enamel defect groups, with 30 children in each group. Salivary vitamin D and MMP-20 levels were measured using ELISA, while ALP was measured using a colorimetric method. Data were analyzed using the Mann–Whitney test. Results: Significant differences in vitamin D (p=0.001) and MMP-20 (p<0.05) levels were observed between the normal enamel and enamel defect groups, whereas ALP showed no significant difference (p=0.641). Conclusion: Salivary vitamin D and MMP-20 levels were significantly associated with enamel defects in children’s permanent teeth, whereas ALP showed no significant association.
