Abstract

Colorectal carcinoma is a malignant disease characterized by abnormal cell proliferation in the colon and rectum. It ranks third globally in cancer incidence, with 1.931.590 cases reported in 2020. Various factors influence its clinical stage, including inflammatory cell proportions in the perivascular area, which can expedite tumor growth, invasion, and metastasis. The neutrophil lymphocyte ratio (NLR) serves as a predictive marker for the clinical stage of patients. An observational analytical study conducted at RSUP Dr. M. Djamil Padang analyzed 141 colorectal carcinoma patients from 2022-2023. The results show majority of patients were male (51%) aged over 50 years (65.95%), with the stage III being the most prevalent (42.55%). Notably, a substantial proportion of patients exhibited high neutrophil counts (>7.5x103/mm3) (60.28%), while lymphocyte counts mostly results within the normal range (1-3.5x10^3/mm3) (61.70%). Receiver Operating Characteristic (ROC) curve analysis to determine the Area Under Curve (AUC) provides a cut off point value of 4.79 with a sensitivity value of 83% and a specificity of 82%. Bivariate analysis with the chi square test between RNL and clinical stages shows p=0.022. Based on this research, there is a relationship between the neutrophil lymphocyte ratio and the clinical stage of colorectal carcinoma patients at RSUP Dr. M. Djamil Padang in 2022-2023.
