Abstract
Pasien neonatus yang dirawat di Neonatus Intensive Care Unit (NICU) sering membutuhkan pemberian obat gentamisin dan nutrisi parenteral (NP) secara bersamaan. Jika pasien menerima infus NP secara kontinue, infus harus dihentikan dan dibilas sebelum pemberian obat lain pada tempat yang sama. Namun proses penghentian tersebut akan menimbulkan masalah dan mempengaruhi keseimbangan cairan tubuh, sehingga koadministrasi antara antara injeksi gentamisin dengan NP dalam infus Y-site sangat diperlukan. Pemberian bersamaan ini perlu mempertimbangkan kajian kompatibilitas fisika sebagai faktor penentu kualitas sediaan parenteral yang berdampak pada keberhasilan terapi pasien di NICU. Pengujian dilakukan dengan membuat simulasi pencampuran antara injeksi gentamisin dengan NP melalui administrasi Y-site dengan perbandingan 1:1, semua pencampuran dilakukan pada kondisi aseptis. NP yang diuji ada 2 formulasi yaitu NP 2-in-1 dan NP 3-in-1 (mengandung lemak). Pada NP 2-in-1 dilakukan pengujian presipitasi, sementara pada pengujian NP 3-in-1 ditambahkan pengujian stabilitas emulsi. Pengujian dilakukan segera setelah pencampuran (T0), 1 jam setelah pencampuran (T1) dan 4 jam setelah pencampuran (T4). Hasil penelitian menunjukkan bahwa gentamisin yang diadministrasikan melalui Y-site dengan NP 2-in-1 dan NP 3-in-1 yaitu tidak adanya partikulat, endapan, turbiditas, perubahan pH, pemisahan fase dan peningkatan ukuran diameter droplet lipid jika infus diberikan dalam waktu 4 jam. Oleh karena itu, pemberian bersamaan dapat dilakukan antara injeksi gentamisin dengan NP 2-in-1 dan NP 3-in-1 melalui administrasi Y-site. ABSTRACT Neonatus patients treated at the Neonatus Intensive Care Unit (NICU) generally require gentamicin and parenteral nutrition (PN) treatment at the same time. If the patient receives a continuous infusion of PN, the infusion should be stopped and cleaned before another treatment at the same place. However, the cessation process will cause problems and affect the balance of body fluids, so the co-administration between gentamicin injection and PN in the Y-site infusion is necessary. This concurrent treatment needs to consider physical compatibility studies as a determinant factor of the quality of parenteral preparations which has an impact on the success of NICU patient therapy. The test was performed by simulating mixing between gentamicin injection with NP in the Y-site infusion at 1: 1 ratio, all mixing was done under aseptic condition. There ware 2 NP tested formulations, NP 2-in-1 and NP 3-in-1 (containing fat). In the NP 2-in-1 precipitation testing was performed, while in the NP 3-in-1 test, an emulsion stability test was added. The test was performed immediately after mixing (T0), 1 hour after mixing (T1) and 4 hours after mixing (T4). The results showed that gentamicin administered via Y-site with NP 2-in-1 and NP 3-in-1 did not contain particulates, precipitation, turbidity, pH change, phase separation and increasing the droplet diameter lipid size if infusion was administered within 4 hour. Therefore, co-administration may be performed between gentamicin injection with NP 2-in-1 and NP 3-in-1 through the administration of Y-site.
