Abstract
ABSTRAK HUBUNGAN POLIMORFISME PROMOTOR DAN EKSPRESI GEN ACTIVATING TRANSCRIPTION FACTOR 3 DAN ANDROGEN RECEPTOR, DENGAN KEJADIAN HIPOSPADIA Yevri Zulfiqar Hipospadia merupakan salah satu kelainan kongenital yang sering dijumpai, tetapi sampai sekarang belum diketahui dengan pasti etiologinya. Seiring dengan perkembangan biologi molekuler dan analisis microarray, hipospadia dikaitkan dengan gangguan dari ekspresi dan polimorfisme genetik. Gen-gen seperti activating transcription factor (ATF3) dan androgen receptor (AR) diduga berhubungan dengan kejadian hipospadia. Penelitian ini bertujuan untuk menganalisis hubungan polimorfisme promotor dan kadar ekspresi dari ATF3 dan AR dengan kejadian hipospadia. Jenis penelitian ini adalah observasional dengan menggunakan desain cross sectional comparative study dengan teknik pengambilan sampel adalah random consecutive sampling. Sampel adalah yang memenuhi kriteria inklusi dan eksklusi dimana kriteria eksklusi pada penelitian ini penderita hipospadia dengan mikropenis dan undesensus testis. Jaringan preputium diambil pada saat operasi.. Untuk pemeriksaan polimorfisme pada ATF3 dan AR digunakan teknik PCR-sequencing dan teknik qPCR untuk pemeriksaan ekspresi gen. Subjek penelitian adalah 49 pasien hipospadia dan 49 pasien sirkumsisi (sebagai kontrol) yang menjalani operasi dari Januari 2014 hingga September 2016. Usia rata-rata pasien dioperasi di rentang 10 - 14 tahun dengan tipe hipospadia terbanyak adalah tipe distal. Hasil penelitian menunjukkan polimorfisme ATF3 pada sampel dan kontrol, tetapi secara statistik tidak bermakna (p>0,05). Ditemukan overekspresi pada gen ATF3 dan AR pada sampel, secara statistik bermakna (p<0.05). Dari hasil penelitian kami, ditemukan bahwa overekspresi dari gen ATF3 dan AR berhubungan dengan kejadian hipospadia. Kata kunci: Hipospadia, Gen activating transcription factor 3 (ATF3), Gen androgen receptor (AR) ABSTRACT THE CORRELATION OF PROMOTER POLYMORPHISM AND EXPRESSION OF ACTIVATING TRANSCRIPTION FACTOR 3, AND ANDROGEN RECEPTOR GENES WITH HYPOSPADIAS INCIDENCE Yevri Zulfiqar Hypospadia is one of the most common congenital disorders, but its etiology is remain indefinite. Along with the advancement of molecular biology and microarray analysis technology, it is suggested that hypospadias are potentially associated with the disorders of genetic expression and polymorphism. Genes such as activating transcription factor (ATF3) and androgen receptor (AR) are thought to be related to hypospadias. The aim of this study was to analyze the correlation of promoter polymorphism and expression of ATF3 and AR with the incidence of hypospadias. The design of this study is observational cross sectional comparative study. We applied random consecutive sampling method and we excluded hypospadias patient with micropenis or and with undecenscus testis. Preputial tissues were taken at the time of surgery. We applied PCR-sequencing techniques for polymorphism examination and qPCR techniques for gene expression examination of ATF3 and AR. Subjects were 49 hypospadias patients and 49 circumcision patients (as control) who underwent surgery from January 2014 until September 2016. Ages of the subjects at surgery were 10 to 14 years old with the most common type of hypospadia was the distal type. ATF3 polymorphism was found in samples and controls, but not statistically different (p>0.005). ATF3 and AR overexpression were found in samples and statistically significant (p<0.005). Our data suggests that ATF3 and AR overexpression correlate with hypospadias incidences. Key words: Hypospadia, Activating Transcription Factor 3 Gene, Androgen Receptor Gene
