Abstract

Background : Ovarian cancer is the most lethal malignancy in the field of gynecology . Usually the cancer is found in an advanced stage with survival rate is less than 30 % . This is due to the lack of adequate ovarian cancer screening. One of ovarian cancer screening that widely accepted is tumor marker CA125. Although it has a good sensitivity, but also increased with benign tumors. Natural Killer Cells is one of the main body's immune response against cancer cells . As a component of the ovarian tumor CA125 was able to trick the immune cells (NK cells) in recognizing the tumor cells. CA125 thus able to suppress the expression of NK cells. Objective: Knowing the sensitivity, specificity, positive predictive value and negative predictive value of the number of NK cells and levels of serum CA125 in patients with ovarian tumors as ovarian tumor malignancy prediction instrument. Method : This study is a diagnostic test at the department of obstetry and gynecology Dr. M. Djamil Hospital in Padang since November 2015 - May 2016, with the number of respondents 40 patients diagnosed with ovarian tumors, which met the inclusion criteria and there is no exclusion criteria . CA125 examination conducted at the Laboratory of Clinical Pathology Dr. M. Djamil Hospital in Padang. NK cells examined using FACS Calibur flowcytometri method in Dharmais Hospital in Jakarta. Result : Results of histopathological examination is used as the gold standard to the point of intersection of NK cell counts < 90 cells / ul obtained sensitivity, specificity , positive predictive value , negative predictive value of the number of NK cells against tumor ovarian malignancies are respectively 55.55 % , 77.27 % , 66.67 % and 68 % . Results of histopathological examination is used as the gold standard to the point of intersection of the levels of CA125 > 35 U / ml obatained sensitivity , specificity , positive predictive value , negative predictive value of CA125 levels to the severity of ovarian tumors are respectively 94.44 % , 36.36 % , 54 , 83 % and 88.88 % . Sensitivity , specificity , positive predictive value , negative predictive value of the combination of the number of NK cells and the CA125 ovarian tumor malignancies are respectively 55.55 % , 50 % , 47.61 % and 61.11 % . Conclusions : The number of NK cells can not be used in predicting malignancy of ovarian tumors . CA125 can be used as predictors of malignancy of ovarian tumors. The combination of NK cells and CA125 can not be used in the prediction of ovarian tumor malignancy. Keywords: ovarian cancer, CA125, natural killer cells, tumor marker. Djamil Padang Latar Belakang : Kanker ovarium merupakan keganasan yang paling mematikan di bidang ginekologi. Biasanya kanker ditemukan dalam stadium lanjut dengan angka ketahanan hidup (ANGKA KETAHANAN HIDUP) kurang dari 30%. Hal ini disebabkan belum adanya skrining kanker ovarium yang benar-benar adekuat. Salah satu skrining kanker ovarium yang diterima luas adalah tumor marker CA125. Walaupun mempunyai sensitifitas yang baik, namun CA125 juga meningkat pada tumor yang jinak. Sel Natural Killer merupakan salah satu respon utama imunitas tubuh melawan sel kanker. Sebagai komponen dari tumor ovarium CA125 ternyata bisa mengelabui sel imun (sel NK) dalam mengenali sel tumor. Sehingga CA125 mampu menekan ekspresi dari sel NK. Tujuan : Mengetahui sensitivitas, spesifitas, nilai duga positif dan nilai duga negatif dari jumlah sel NK dan kadar CA125 serum pada penderita tumor ovarium sebagai instrumen prediksi keganasan tumor ovarium. Metode Penelitian : Penelitian ini merupakan penelitian uji diagnostik di bagian obstetri dan ginekologi RSUP Dr. M. Djamil Padang sejak November 2015-Mei 2016, dengan jumlah responden 40 pasien yang didiagnosis dengan tumor ovarium, yang memenuhi kriteria inklusi dan tidak didapatkan kriteria eksklusi. Pemeriksaan CA125 dilakukan di Laboratorium Patologi Klinik RSUP Dr.M.Djamil Padang. Sel NK diperiksa dengan metode flowcitometri menggunakan FACS Calibur di RS. Dharmais Jakarta. Hasil : Hasil pemeriksaan histopatologi digunakan sebagai standar emas dengan titik potong jumlah sel NK <90 sel/ul didapatkan sensitivitas, spesifisitas, nilai duga positif, nilai duga negative dari jumlah sel NK terhadap keganasan tumor ovarium berturut-turut adalah 55,55%, 77,27%, 66,67%, dan 68%. Hasil pemeriksaan histopatologi digunakan sebagai standar emas dengan titik potong kadar CA125>35 U/ml Sensitivitas, spesifisitas, nilai duga positif, nilai duga negative dari kadar CA125 terhadap keganasan tumor ovarium berturut-turut adalah 94,44%, 36,36%, 54,83% dan 88,88%. Sensitivitas, spesifisitas, nilai duga positif, nilai duga negative dari kombinasi jumlah sel NK dan CA125 terhadap keganasan tumor ovarium berturut-turut adalah 55,55%, 50%, 47,61%, dan 61,11%. Kesimpulan : Jumlah sel NK tidak dapat digunakan dalam memprediksi keganasan tumor ovarium. CA125 dapat digunakan sebagai prediksi keganasan tumor ovarium. Kombinasi sel NK dan CA125 tidak dapat digunakan dalam prediksi keganasan tumor ovarium. Kata Kunci : kanker ovarium, CA125, sel natural killer, tumor marker.
