Abstract

Background and purpose: Obesity is a major risk factor for stroke, particularly ischemic stroke. However, its effect on ischemic stroke outcomes is less clear. Many studies have shown better cognitive and functional status outcomes in ischemic stroke patients with obesity. This review aims to investigate the BMI’s effect on the cognitive and functional status outcomes of ischemic stroke patients. Method: The reviewers searched Pubmed and Google Scholar from inception to December 2020. Studies were eligible if they discuss BMI’s effect on the cognitive and functional status of ischemic stroke patients with a cohort study design. Results and Conclusions: Most studies demonstrated the BMI's effect on cognitive and functional status outcomes in ischemic stroke patients. BMI's effect on the results of cognitive status was influenced by age, it was suspected that obese patients in middle age had leptin resistance so that ischemic stroke patients with obesity in middle age got worse cognitive results. BMI's effect on functional status was reported by ten studies that was overweight/obese ischemic stroke patients had better functional outcomes. This obesity paradox may be due to the protective role of adipose tissue. Higher-quality studies were needed to clarify this effect considering the methodological limitations of previous studies.
