Abstract
Being one of Filariasis endemic areas in West Sumatera Province, mass treatment is needed and has been conducted in Agam District since 2005. Nevertheless, Transmission Assessment Survey undertaken six years after mass treatment found that 7.7% of elementary school students were still positively infected by filarial worm. It was also revealed that the coverage of medication intake did not achieve the target of 85% of the population who accepted the drugs. These results indicate that intervention for health cadres is necessary due to their important role in drugs distribution and program socialization during mass treatment. Therefore the aim of this study is to develop an intervention model to increase public compliance in taking the medications. This study employed the mixed method of quantitative and qualitative approach. In quantitative method, a cross-sectional design was used to assess the medication coverage and quasi-experimental was conducted to determine the impact of intervention (refreshing) among health cadres. The qualitative research with in-depth interview method was employed to reveal the existence of social capital existence in mass treatment program for filariasis prevention. This study revealed that the knowledge level of cadres increased by 26.8% after refreshing program (p value = 0.001). The cadres attitude was also increased but the difference was not statistically significant (p value = 0.201). In-depth interviews revealed that social capital do exist in the implementation of mass treatment. Medication adherence was higher in the intervention areas (18.9%) compared to the control areas (13.1%). The increasing of medication adherence was significant in intervention areas (p value = 0.001) and it was not significant in control area (p value = 0.093). It can be concluded that refreshing among cadres with social capital strengthening will increase the cadres’ knowledge and attitude. It finally will contribute to the improvement of medication adherence during mass treatment program. The district health office of Agam should take the advantage of existing social capital that was growing up in the community to enhance the medication coverage, which lead to the effectiveness of mas treatment program.
