Abstract
Backgroud. Perforation is the most serious complication in patients with perforated peptic ulcer (PPU). The mortality rate due to PPU varies from 1.3% to 82.4%. Data and research on predictive factors of mortality in PPU patients and assessment of the accuracy of the Boey, ASA, and qSOFA scores as predictors of mortality show varying results and have not been widely carried out in Indonesia. Method. This study is an observational analytic study with a cross sectional design and a diagnostic test approach. This research was conducted at the Central General Hospital Dr. M. Djamil Padang. The sample size in this study was 72 research subjects. Result. A total of 59.7% of the patients were male and the mean age of the research subjects was 64.8 years. The mortality rate during the treatment period in the subjects of this study was 52.8%. Bivariate analysis showed that there was a significant relationship between age (p= 0.029), duration of surgery (p= 0.040), preoperative shock (p= 0.049), preoperative creatinine level (p= <0.001), Boey score (p= 0.002), score ASA (p=0.001), and qSOFA (p= <0.001) with mortality. Multivariate analysis showed that there was a significant relationship between age (p= 0.029), preoperative creatinine level (p= 0.010), and qSOFA score (p= 0.001) and mortality. The ROC curve shows the AUC score of Boey's score is 0.73, the ASA score is 0.69, the qSOFA score is 0.77, and the M. Djamil predictor model is 0.89. Conclusion. The factors that have the strongest relationship with the mortality of PPU patients in Dr. M. Djamil Hospital were age, preoperative creatinine level, and qSOFA score. The M. Djamil predictor model has the best level of accuracy in predicting mortality in PPU patients compared to Boey, ASA, and qSOFA scores.
