Abstract

Latar belakang: Sumber Daya Manusia Puskesmas, sarana prasarana dan ketersediaan obat di Puskesmas Kota Payakumbuh belum memadai. Terjadi peningkatan kunjungan sakit ke Puskesmas dan kasus medis yang menjadi kompetensi Puskesmas belum tuntas. Tujuan penelitian ini yaitu untuk mengetahui pelaksanaan Program Jaminan Kesehatan Nasional sesuai Peraturan Menteri Kesehatan Nomor 28 Tahun 2014 di Puskesmas Kota Payakumbuh. Metode: penelitian menggunakan pendekatan kualitatif dan merupakan studi kebijakan (policy study) dengan analisis restropektif. Informan penelitian berjumlah 14 orang. Teknik pengumpulan data melalui observasi, wawancara semiterstruktur dan telaah dokumen. Analisa data dilakukan sebelum, selama dan sesudah di lapangan tempat penelitian. Hasil: pada komponen masukan yaitu Sumber Daya Manusia Puskesmas, pengelolaan dana kapitasi, sarana prasarana dan ketersediaan obat dalam pelaksanaan program Jaminan Kesehatan Nasional belum sesuai peraturan. Komponen proses yaitu peserta dan pelaksanaan pelayanan kesehatan belum sesuai peraturan, namun mekhanisme penanganan keluhan sudah terlaksana sesuai peraturan. Komponen keluaran yaitu penapisan rujukan ke Fasilitas Kesehatan Tingkat Lanjut tidak melebihi target dan sudah sesuai ketentuan yang berlaku. Kesimpulan: pelaksanaan program Jaminan Kesehatan Nasional di Puskesmas Kota Payakumbuh tahun 2015 belum seluruhnya sesuai peraturan yaitu pada komponen masukan dan komponen proses. The purpose of this study is to investigate the implementation of the National Health Insurance Program according to the Minister of Health No. 28 in 2014 of PHC Payakumbuh. Methods: The study used a qualitative approach and the policy study with retrospective analysis. Informants research amount to 14 people. The technique of collecting data through observation, semi-structured interviews and document review. Data analysis is done before, during and after field research site. Results: the input component is PHC Human Resources, capitation funds management, means of pre facilities and availability of medicines in implementation National Health Insurance program not according to the rules. The process component that participants and implementation of health services does not meet the regulations, but mechanism handling of complaints already implemented according to the rules. Component output is screening referrals to health facilities Advanced does not exceed the target and is in conformity with prevailing regulations. Conclusion: the implementation of the National Health Insurance program at the public health center Payakumbuh 2015 has not been entirely according to the rules that the input component and the process component.
