Abstract

Risk management is a way of doing to know, analyze and control risk in every activity of company either sector of product or service with aim to get more effectivity and efficiency. One of the ISO studies on risk management has been guided in ISO 31000 (2009). This ISO provides a standard framework with the title "Risk Management-Principles and Guidelines on Implementation". According to sources from the Ministry of Health in 2016, there are 2,488 hospitals spread throughout Indonesia. However, only 10.29% of hospitals are able to run the Wastewater Treatment Plant (WWTP) properly. The purpose of this research is to determine risk priority of Wastewater Treatment Installation at Ibnu Sina Hospital, Padang and priority risk evaluation of Wastewater Treatment Plant (WTP) at Ibnu Sina hospital, Padang. The method used in this research is fuzzy AHP, the method of changing the questionnaire results obtained from the Ibnu Sina Islamic hospital, Padang from qualitative to quantitative, before the questionnaire was given to the hospital, validation of the questionnaire to the environmental department of Padang, as the party monitor and monitor the results of waste generated at all times by the hospital. 5 priority risk among the first is the risk of pipe with substraction of liquid waste stuck with a weight value of 0,0779, the second is septictank risk with excess nitrification subrisiko with a weight value of 0,0713, the third is the risk of sampit with subcritical liquid waste spilled with a weight value of 0,0642, the fourth is the risk of human resources with the subcredit of the lack of quality with the weight value of 0,0590 and the fifth risk is the blower with the subcancer condition is not feasible to use with the weight value of 0,0548.
