Abstract

ABSTRAK HUBUNGAN POLIMORFISME GEN PARK2 SNP PARK2-e01(-2599) DAN SNP rs1040079 DENGAN KEJADIAN PENYAKIT KUSTA BERDASARKAN KADAR NUCLEARFACTOR KAPPA BETA DAN LAMININ ALFA2 Qaira Anum Faktor genetik penting dalam menentukan seseorang untuk tertularnya penyakit kusta. Salah satu gen yang memengaruhi kerentanan terhadap M. leprae adalah gen PARK2 SNP PARK2-e01(-2599) dan SNP rs1040079. Gen ini berfungsi sebagai fagositosis pada makrofag dalam proses imunitas non spesifik. NF-kβ mempunyai peranan penting dalam respon imun sebagai mediator proinflamasi. Laminin-α2 merupakan mediator spesifik M.leprae yang berikatan dengan sel Schwann saraf perifer. Penelitian ini bertujuan untuk membuktikan terdapat gen PARK2 SNP PARK2-e01(-2599) dan SNP rs1040079 pada pasien kusta dan narakontak. Menganalisis hubungan polimorfisme gen PARK2 SNP PARK2-e01(-2599) dan SNP rs1040079 dengan kejadian penyakit kusta berdasarkan kadar NF-kβ dan laminin-α2. Penelitian ini merupakan penelitian observasional dengan disain cross sectional comparative study. Pengambilan sampel dengan consecutive sampling terhadap pasien kusta dan narakontak yang datang berobat ke poliklinik Kulit dan Kelamin RS Dr. M. Djamil Padang, puskesmas Kabupaten Padang Pariaman, Kota Pariaman dan Kabupaten Limapuluh Kota di Provinsi Sumatera Barat. Kasus adalah subjek masyarakat Sumatera Barat penderita kusta sebanyak 30 orang dengan kontrol juga sebanyak 60 orang. Tahapan penelitian adalah pengumpulan sampel, isolasi DNA, disain primer ke-2 SNP, RFLP-PCR, PCR metode direct sequencing dan analisis. Hasil penelitian ini didapatkan umur terbanyak pasien kusta adalah 12-16 tahun dan narakontak umur 36-45 tahun (p=0,293). Kelompok pasien kusta didapatkan 90% yang memiliki polimorfisme SNP SNP PARK2-e01(-2599) sedangkan narakontak yang memiliki polimorfisme SNP PARK2-e01(-2599) sebanyak 83,3%. Hasil uji statistik diperoleh p=0,75 dengan OR=1,8(95%CI 0,389-8,323). Kelompok pasien kusta yang memiliki polimorfisme SNP rs1040079 sebanyak 23,3%, sedangkan narakontak yang memiliki polimorfisme SNP rs1040079 adalah 33,3%. Uji statistik p=0,567 dengan OR-0,609(95%CI 0,195-1,897). Nilai rerata kadar NF-kβ ada kelompok pasien kusta 9,28 ng/ml, sedangkan pada narakontak adalah 9,32 ng/ml. uji statistik p=0,924. Nilai rerata kadar laminin-α2 pada kelompok kusta adalah 18,27 ng/ml sedangkan pada narakontak adalah 16,78 ng/ml. uji statistik p=0,587. Hubungan Polimorfisme gen PARK2 SNP PARK2-e01(-2599) dengan kejadian penyakit kusta berdasarkan kadar NF-kβ dan laminin-α2 didapatkan nilai p=0,953. Hubungan polimorfisme gen PARK2 SNP rs1040079 dengan terjadinya penyakit kusta berdasarkan kadar NF-kβ dan laminin-α2 didapat uji statistik p=0,550. Hasil penelitian ini dapat disimpulkan bahwa terdapatnya polimorfisme gen PARK2 SNP PARK2-e01(-2599) dan SNP rs1040079 pada pasien kusta dan narakontak. Polimorfisme gen PARK2 SNP PARK2-e01(-2599) dan SNP rs1040079 tidak berhubungan dengan kejadian penyakit kusta berdasarkan kadar NF-kβ dan laminin-α2. Namun terdapat kecenderungan risiko kejadian penyakit kusta. Kata kunci : Kejadian penyakit kusta, polimorfisme gen, NF-kβ, laminin-α2 ABSTRACT The Correlation of PARK2 SNP PARK2-e01(-2599) Gene Polymorphism and rs 1040079 with Leprosy Occurrence Based on Nuclear Factor Kappa Beta and Laminin Alfa 2 Levels Qaira Anum Genetic factor is important in determining a person to get leprosy. Genes that affect susceptibility to Mycobacterium leprae are PARK2 SNP PARK2-e01(-2599) and SNP rs 1040079 genes. These genes act as a phagocytosis in macrophages in a non-specific immune process. NF-kβ has an important role in the immune response as a proinflammatory mediator. Laminin-α2 is a specific mediator of Mycobacterium leprae that binds to peripheral Schwan cells. This study aims to prove that there are PARK2 SNP PARK2-e01(-2599) and SNP rs 1040079 genes in leprosy patients and household contact. Analyzes the relationship of PARK2 SNP PARK2-e01(-2599) polymorphism gene and SNP rs1040079 with leprosy occurrence based on NF-kβ and laminin-α2 levels. This is an observational research with cross sectional comparative study design, consecutive sampling of leprosy patients and household contact came to Outpatient Clinic of Dermatology and Venereology Department, Dr. M. Djamil hospital Padang, Public Health Center of Padang Pariaman, Pariaman city and Limapuluh Kota district of West Sumatera. The subjects are 30 peopl, control 60 people. The research stages are sample collection, DNA isolation, primary design for SNP, RFLP-PCR, PCR direct sequencing method and analysis. The results of this study obtained the most aged patients leprosy was 12-16 years and household contact aged was 36-45 years (p=0.293). The leprosy patients group who had PARK2 SNP PARK2-e01(-2599) polymorphism gene was 90% while in household contact was 83.3%. Statistical test results obtained p=0.75 with OR=1.8 (95% CI 0.389-8.323). The leprosy patients group had SNP rs1040079 polymorphisms gene was 23.3%, whereas the household contact was 33.3%. Statistical test p=0,567 with OR-609 (95% CI 0,195-1,897). The mean value of NF-kβ levels in the leprosy patients group was 9.28 ng/ml, whereas in household contact was 9.32 ng /ml. Statistic test p=0,924. The mean value of laminin-α2 level in leprosy group was 18,27 ng/ml while in the household contact was 16,78 ng ml. Statistic test p = 0,587. The relationship of PARK2 SNP PARK2-e01(-2599) polymorphism gene with the incidence of leprosy disease based on levels of NF-kβ and laminin-α2 obtained p=0.953. The relationship of PARK2 gene polymorphism with the incidence of leprosy disease based on levels of NF-kβ and laminin α2 obtained statistical test p = 0,550. The results of this study can be concluded that there was a presence of PARK2 SNP PARK2-e01 (-2599) polymorphism and SNP rs 1040079 genes in patients with leprosy and household cntact. PARK2 SNP PARK2-e01 (-2599) and SNP rs1040079 polymorphism gene was not associated with leprosy occurrence based on NF-kβ and Laminin-α2 levels. But there is potential risk to leprosy occurrence.
