Abstract

KORELASI INDEKS HEMOLISIS ERITROSIT DENGAN KADAR 2,3 DIFOSFOGLISERAT PACKED RED CELL SELAMA PENYIMPANAN DI BANK DARAH ABSTRAK Latar Belakang: Packed red cell (PRC) adalah komponen darah yang dipisahkan dari whole blood sehingga dapat disimpan pada kondisi optimalnya. Penyimpanan PRC diperkirakan menyebabkan storage lesion diantaranya peningkatan indeks hemolisis eritrosit dan penurunan kadar 2,3 difosfogiserat/diphosphoglycerate (2,3 DPG). 2,3 difosfogliserat adalah fosfat anorganik yang diproduksi di dalam eritrosit dan berfungsi menurunkan afinitas hemoglobin terhadap oksigen. Hemolisis eritrosit diperkirakan menyebabkan penurunan kadar 2,3 DPG. Tujuan penelitian ini untuk mengetahui korelasi antara indeks hemolisis eritrosit dengan kadar 2,3 DPG pada unit PRC selama penyimpanan. Metode: Penelitian ini merupakan penelitian analitik dengan rancang potong lintang terhadap 14 unit PRC yang memenuhi kriteria inklusi dan ekslusi. Penelitian dilakukan pada November 2016- Mei 2017. Pemeriksaan dilakukan selama 28 hari penyimpanan PRC. Pemeriksaan parameter untuk menghitung indeks hemolisis eritrosit menggunakan rumus indeks hemolisis. Pemeriksaan kadar 2,3 DPG dilakukan dengan metode enzyme linked immunosorbent assay (ELISA). Data dianalisis dengan menggunakan uji Spearman. Nilai bermakna jika p<0,05. Hasil: Rerata indeks hemolisis eritrosit dan kadar 2,3 DPG PRC selama penyimpanan adalah 0,20% dan 1,13 μmol/mL masing-masingnya. Korelasi indeks hemolisis eritrosit dengan kadar 2,3 DPG adalah 0,49 selama penyimpanan PRC dengan p<0,05 yang bermakna secara statistik. Simpulan: Indeks hemolisis eritrosit berkorelasi negatif sedang dengan kadar 2,3 DPG selama penyimpanan PRC. Kata Kunci: Packed red cell, Red blood cell storage lesions, hemolisis , 2,3 DPG CORRELATION BETWEEN ERYTHROCYTE HEMOLYSIS INDEX AND 2,3 DIPHOSPHOGLYCERATE IN PACKED RED CELL DURING BLOOD BANK STORAGE PERIOD ABSTRACT Background: The packed red cell (PRC) is blood component that has separate from whole blood (WB) and can be storage in the optimal condition. The packed red cell storage is predicted can cause storage lesions that are increasing of erythrocyte hemolysis index and decreasing 2,3 diphosphoglycerate (2,3 DPG) level. The 2,3 DPG is anorganic phosphate produce by erythrocyte and it can decrease hemoglobin affinity to oxygen. The erythrocyte hemolysis is predicted can decrease 2,3 DPG level. The aim of this study is to determine correlation of erythrocyte hemolysis index with 2,3 DPG level in PRC units during storage. Methods: This was an analytic study with cross sectional design that has inclusion and exclusion criteria. The study was conducted from November 2016 to May 2017. The test were performed on 28 days of PRC storage. The formula hemolysis index was used to calculated erythrocyte hemolysis index. The examination 2,3 DPG levels were perfomed with enzyme linked immunoassay sorbent assay (ELISA) method. Data analyzed using statistic Spearman test with p value < 0,05 is significance difference. Results: The mean of erythrocyte hemolysis index and 2,3 DPG levels on 28 days of PRC storage time are 0,20% and 1,13 μmol/mL respectively. The correlation of erythrocyte hemolysis index with 2,3 DPG levels is 0,49 in PRC during storage. That has significantly difference with p value < 0,05. Conclusion: The correlation between erythrocyte hemolysis index and 2,3 DPG levels in during storage of PRC is a moderate negative correlation.
