Abstract

WHO (2012) classify the stages of shock in Dengue Shock Syndrom (DSS) into compensated and decompensated. Poor perfusion in DSS resulting in tissue hypoxia, and shock produce more lactate. Blood lactate levels could be used as a marker to determine the severity of DSS Objective Aim of this study is determine the differences in serial blood lactic compensated and decompensated shock in DSS. Method This is a cross sectional study conducted on January 2016 to January 2017 at Pediatric departement Dr.M Djamil Hospital. Samples were collected using consecutive sampling method.. The samples were divided into two groups based on clinical type of shock. Lactate was examined serially using Accutrend lactate meter that is on hour 0 (L1), hour 6 (L2), hour 12 (L3) and hour 24 (L4).Hyperlactatemia are > 2mmol / L. Data was analyzed using independent t-test. Result There were 40 samples divided equally in to each group. Lactate level highest in L1. In compensated group mean blood lactate was increased 4.7 + 0.97 mmol / L and also in decompensated group was 6 + 1.64 mmol / L. There were significan differences in mean blood lactate level on hour 0 ( L1) between two group (p,<0.05). Conclutions The mean levels of blood lactate in the DSS group decompensation higher than compensated group,significant differences obtained at the beginning of the reception at the hospital, hour 0 ( L1).
