Abstract

Decreased levels of hemoglobin and increased levels of reticulocytes indicate the presence of hemolysis process on patients with autoimmune hemolytic anemia (AIHA) while the autoimmune component is indicated through examination of the Direct Coombs Test (DCT) with gradation +1, +2, +3 and +4 as the result. Previous studies have noted that the higher gradation of DCT, the greater degree of hemolysis. This study aims to determine the mean differences of hemoglobin levels and reticulocyte levels with gradation of direct coombs test on autoimmune hemolytic anemia patients. This study used cross sectional design in population of patients with newly diagnosed AIHA. This is an analytic research using secondary datas which is medical records of 106 patients with AIHA at Dr Dr. M. Djamil Padang from January 2012 - June 2016 were taken in sequence. The statistical analysis used is the One-Way ANOVA and Kruskal-Wallis test. The analysis showed a mean hemoglobin level was 6.74 ± 2.01 g / dl, the median reticulocyte level was 4,17 (1,7-28) % and was found AIHA patients with positive gradation DCT 1 (+1) 54.7%, positive DCT 2 (+2) 17%, positive DCT 3 (+3) of 22.6%, and patients with positive DCT 4 (+4) 5.7%. It showed a significant difference (p<0.05) in each value of differences between the mean levels of hemoglobin and reticulocytes with various gradations of DCT in patients with AIHA. It is concluded that there is a significant relationship between decrease levels of hemoglobin with the severity of gradation DCT and there is a significant relationship between increase levels of reticulocytes with the severity of gradation DCT in patients with AIHA.
