Abstract

SKALA SIGMA PARAMETER KIMIA KLINIK DI RSUP. DR. M. DJAMIL PADANG ABSTRAK Latar Belakang: Skala sigma menggambarkan probabilitas kesalahan yang dapat berisiko buruk pada pasien jika pemeriksaan sampel berjalan sesuai prosedur. Skala sigma laboratorium yang tinggi dapat memberikan hasil pemeriksan laboratorium yang terpercaya, meningkatkan efisiensi penggunaan bahan kontrol dan meminimalkan aturan Westgard yang dipakai. Penelitian ini bertujuan untuk menghitung skala sigma alat kimia klinik di laboratorium sentral RSUP. Dr. M. Djamil. Metode: Penelitian ini adalah penelitian deskriptif retrospektif dengan menghitung skala sigma berdasarkan data total allowable error, bias dan koefisien variasi pada alat kimia klinik Cobas Integra 400 Plus dan Architect C8000. Hasil: Skala sigma terhadap 10 parameter alat Cobas Integra 400 Plus didapatkan 1 parameter dengan sigma >3 dan 9 parameter dengan sigma ≤3. Skala sigma terhadap 3 parameter alat Architect C8000 level 1 didapatkan 1 parameter dengan sigma >3. Skala sigma Architect C8000 level 2 didapatkan 9 parameter dengan sigma ≤3. Skala sigma Architect C8000 level 3 didapatkan 1 parameter dengan sigma >3 dan 8 parameter dengan sigma ≤3. Analisis Quality Goal Index harus dilakukan pada parameter dengan skala sigma ≤3 untuk mencari penyebab skala sigma parameter tersebut rendah. Simpulan: Penerapan skala sigma di laboratorium dapat membantu menilai performa kinerja alat laboratorium, mengatasi penyebab performa kinerja yang buruk dan optimalisasi aturan Westgard Kata Kunci: Bias, Koefisien Variasi, Total Allowable Error, Skala Sigma, Quality Goal Index SIGMA METRICS OF CLINICAL CHEMISTRY PARAMETERS IN DR. M. DJAMIL PADANG HOSPITAL ABSTRACT Background: The sigma metric describes the probability of error that can be a bad risk to the patients if the sample examination is carried out according to the procedure. High sigma scale in laboratory can provide reliable laboratory test results, increasing the efficiency of control materials and reduce the Westgard rules. This study aims to calculate the sigma scale of clinical chemistry analyzers in Dr. M. Djamil Hospital. Methods: This study is a retrospective descriptive study by calculating the sigma scale based on the total allowable error, bias and coefficient variation data on the Cobas Integra 400 Plus and Architect C8000 clinical chemical analyzers. Results: The sigma scale of 10 parameters of Integra Cobas 400 Plus was found >3 for 1 parameter and sigma ≤3 for another 9 parameters. The sigma scale of 3 parameters of Architect C8000 was found >3 analyzed in first level. For the second level, the sigma scale of 9 parameters of Architect C8000 was found ≤3. The sigma scale of 1 parameters of Architect C8000 was found >3 and ≤3 for another 8 parameters analyzed in third level. Analysis of the Quality Goal Index must be carried out on parameters with sigma ≤3 to find out the cause of the low sigma. Conclusion: The application of the sigma scale in the laboratory can help assess the performance of laboratory equipment, address the causes of poor performance and optimize Westgard's rules.
